Dear Colleagues, The American Cancer Society’s most recent estimations indicate 1 in 3 women and 1 in 2 men will develop cancer in their lifetimes. Earlier this year, the Society predicted 1,658,370 new cases and over half a million deaths this year alone. As the world population grows, so does the number of afflicted individuals, and those among them that would develop critical illnesses requiring intensive care. We need to prepare for this anticipated influx; however, we do not have a good understanding of the current critical care needs of this population or the dedicated resources at our disposal. As members of the critical care community, we have a responsibility to address these challenges by identifying the problems, and developing large scale and viable solutions. Among the most important areas needing improvement are the ethical dilemmas we confront regarding admission to the intensive care unit on a daily basis, end-of-life discussions, and advance care planning in all patients, prevention of critical illnesses, improvement of short and long-term outcomes, the accuracy of the prediction models, and most importantly, educating our colleagues to improve access and delivery of critical care to cancer patients. To adequately respond to these challenges, we need an extensive and intense collaborative effort that unites oncologic critical care researchers and clinicians from around the world. The Oncologic Critical Care Research Network (ONCCC-R-NET) provides a platform and leadership to successfully achieve these goals. Today, we have begun to close the gaps in a coordinated global effort to fight critical illness in cancer patients. Please join us in this endeavor! Joseph L. Nates, M.D., M.B.A., C.M.Q., M.C.C.M.
